DEADLINE FOR NOMINATIONS: FRIDAY, JANUARY 16, 2009, 5pm Pacific

Eligibility

  1. At least one of the following two criteria must be met by the nominated individual:
    • Nominees must have made a remarkable research, policy, or advocacy contribution with the use of tobacco industry documents, and/or;
    • Nominees must have employed innovative, creative approaches to the employment of tobacco industry documents that result in an improvement in the health or public awareness of a community or nation.
  2. Nominees must not have any affiliation with the tobacco industry.

Any individual meeting the above eligibility criteria and the qualifications set forth in the following guidelines may be nominated, including but not limited to educators, social workers, documents researchers, public health practitioners, legal researchers, grassroots organizers, activists, lawyers, academic and scientific researchers, government officials, physicians, and staff of community-based organizations (CBOs).

Guidelines

Nominations may be made by individuals such as colleagues, peers, coworkers, instructors/professors, governments, or community-based organizations that are qualified to make such a nomination because of their familiarity with the nominee's work and contribution.

Individuals may not nominate more than one person for any one award; however, they may submit a separate nomination for each of the two categories.

Nominations should be made according to the application outline contained in the Call for Nominations and must be accompanied by a minimum of two letters of support. One of the letters of support must be from someone other than the direct supervisor of the nominee or any employee of the organization/company for which the nominee is a member of the board of directors.

The nominator must answer all questions posed in the application outline found on the Instructions page.

The preferred method of submission is through the Legacy Awards website. The website can be accessed through the Instructions page referenced above.

Completed applications will be acknowledged electronically.

Incomplete applications will not be considered.

Applications received after the deadline (Friday, January 16, 2009, 5pm Pacific) will not be considered and will not be returned.

Members of the awards committee may not nominate potential recipients, although they may provide written support as part of a nomination package.

Employees and directors of the American Legacy Foundation and their relatives are not eligible for either award.

Terms of Award

The selection of the Legacy Tobacco Industry Documents Award recipients will be made by a distinguished awards committee comprised of nationally recognized individuals in the tobacco control and related fields. Each recipient will receive a $7,500 cash award (given to the recipient or an institution or organization upon the recipient's request), a plaque, name inscription at the Center for Tobacco Control Education and Research at the University of California, San Francisco, and attendant media coverage. Award recipients will be expected to attend the awards ceremony at the National Conference on Tobacco or Health to be held June 11, 2009, in Phoenix, AZ.
