Nominate a Deserving Colleague for Community Leadership Award
The 2013 Legacy Community Activist Award nomination deadline has passed. Please come back next summer to apply.
Each year, Legacy recognizes an individual committed to tobacco prevention and cessation at the community level with an award for spearheading innovative and influential tobacco control projects, especially those that reflect our mission. The winner will receive an honorarium of $2,500 as well as recognition through media outreach and Legacy publications.
Nominee Requirements:
- 18 years or older
- A minimum of three years active work in the field of tobacco control
- Involvement in spearheading innovative tobacco control activities
- Recognition as a leader in his/her community*
- Reflection of Legacy’s mission in community activities

*Note: For the purpose of this award, "community" is a geographically-defined locality such as a town, county or region. Focus on a special population within a community may also be considered.
